IN SHORT
Stromal vascular fraction (SVF) is a mix of cells extracted from your own fat after mini-liposuction, often sold as ‘fat stem cells’. US courts confirmed in 2024 that SVF is a drug needing FDA approval, and no SVF treatment is approved.
What is fat-derived stem cells (svf)?
SVF is the cell-rich portion separated from fat tissue. It contains a small share of mesenchymal stromal cells plus blood vessel, immune and other cells. Clinics market it for joints, lungs, nerves and anti-ageing. Fat-derived cells can also be grown in a lab to make larger MSC doses.
How it works
A small amount of fat is removed by liposuction, usually from the belly, under local anaesthetic. The fat is broken down with enzymes or mechanical processing, then spun to isolate the SVF. The cells are injected into a joint or other site, or sometimes given by IV, on the same day. A related approach, micro-fragmented fat, keeps the tissue in small clusters instead of isolating cells.
Legal status
In the US, the FDA treats SVF as more than minimally manipulated, so it is a drug (Section 351) needing approval. In September 2024 the Ninth Circuit ruled for the FDA, holding that SVF differs from the fat removed and does not qualify for the ‘same surgical procedure’ exception; in October 2025 the Supreme Court declined to hear the appeal, leaving that ruling in place.
An earlier Florida case also backed the FDA: a federal district court ruled in 2019 and the Eleventh Circuit upheld it in 2021. In the EU, enzyme-processed SVF is generally treated as an advanced therapy medicinal product.
What the evidence shows
Most SVF evidence is in knee arthritis. Systematic reviews from 2022 to 2025 found mostly small, short studies with a high risk of bias, reporting pain improvement but rarely with good placebo control.
A 2026 network meta-analysis of randomised trials ranked SVF highly but found its improvements over hyaluronic acid did not reach a clinically meaningful level. A 2024 meta-analysis suggested lab-grown fat MSCs gave faster pain relief than SVF, showing products are not interchangeable. There is no good evidence for SVF in lung, neurological or anti-ageing uses.
Commonly marketed for
- Knee and hip arthritis
- Back pain
- COPD and other lung disease
- Neurological conditions such as MS or Parkinson’s
- Erectile dysfunction
- Anti-ageing and cosmetic uses
Cost
Typical price range: $1,150 – $12,000. A 2017 survey of 317 US clinics found stem cell knee injections, including fat-derived procedures, cost $1,150 to $12,000 (average $5,156). Insurance does not cover SVF, which is unapproved.
Risks
- Liposuction complications such as bruising, infection or, rarely, serious bleeding
- Infection from cell processing
- Pain or swelling after injection
- Vision loss has been reported after unapproved SVF eye injections
- Unknown long-term effects of injected processed cells
Questions to ask first
- Given the 2024 court ruling, is this SVF being given under an FDA-authorised trial?
- Is the fat processed with enzymes or kept as tissue, and who does the processing?
- What are the risks of the liposuction step for me?
- What randomised trial evidence supports SVF for my condition?

Fat-Derived Stem Cells (SVF): FAQs
Is SVF legal in the US?
The Ninth Circuit ruled in 2024 that SVF is a drug subject to FDA rules, and the Supreme Court declined to review it in October 2025. Selling it as a treatment outside an approved trial is not lawful.
Isn’t it my own tissue?
Yes, but the court found that processing fat into SVF creates a different product from what was removed. That is why it needs FDA approval.
Does SVF work for knee arthritis?
Small studies report pain relief, but high-quality evidence is limited and benefits over standard injections may not be clinically meaningful.
What are the main risks?
Risks come from both liposuction and injection, including infection and pain. Serious harms, such as vision loss after eye injections, have been reported.
Is micro-fragmented fat the same as SVF?
Not exactly. It keeps fat in small clusters without enzyme digestion, which can affect how it is regulated. Evidence for it is also limited.
