Stem Cell Therapy for Back Pain & Degenerative / Bulging Disc: Evidence, Cost & Trials (2026)

About this guide: We are independent patient researchers, not medical doctors. We review peer-reviewed literature (PubMed), government trial registries and regulators’ guidance, plus documented patient reports, to help you navigate clinic options and spot predatory marketing. How we research.

Many cases of long-lasting low back pain are linked to wear of the spinal discs, the cushions between the vertebrae, which can dry out, crack or bulge. Researchers are testing stem cells injected directly into a damaged disc in the hope that they can calm inflammation and slow or partly reverse disc degeneration.

Does stem cell therapy work for back pain?

Several randomised trials have injected mesenchymal stromal cells into painful degenerated discs, mostly in carefully selected patients with one or two damaged discs. A recent systematic review and meta-analysis of 7 RCTs (594 patients) found modestly lower pain scores and about a 4-point improvement in disability scores compared with sham or control, with similar rates of side effects.

The authors cautioned that these differences may be too small to matter much in daily life. Studies use different cell types and doses, and follow-up beyond 2–3 years is limited. Evidence for bulging or herniated discs, sciatica, or general ‘back pain’ is far weaker than for single-level disc degeneration. Larger, longer trials are under way.

Is any stem cell treatment approved for back pain?

No stem cell product is approved by the US FDA or the EU’s EMA for back pain or disc disease, and the FDA specifically lists back pain among conditions with no approved regenerative therapy. Disc cell therapies remain investigational and are only available legitimately through clinical trials.

What private clinics offer — and how it compares

Clinics often market spinal or disc injections of bone marrow, fat or birth-tissue products as a way to ‘heal discs’ and avoid spine surgery, sometimes treating several levels at once. Trials so far show at best a small average benefit in specially selected patients, and no proof of disc regrowth or of avoiding surgery.

Proven treatments to discuss first

Before paying for an experimental option, make sure you have explored the established treatments for back pain with your doctor:

  • Staying active and guided exercise or physiotherapy
  • Pain relief medicines such as NSAIDs for short periods
  • Psychological approaches such as cognitive behavioural therapy for chronic pain
  • Spinal manipulation, massage or acupuncture for some patients
  • Epidural steroid injections for nerve-root pain (sciatica)
  • Spine surgery for specific problems such as nerve compression with weakness

How much does stem cell therapy for back pain cost?

Private clinics typically quote $1,200 to $28,000. No back-specific US price survey was found; a 2021 UC Irvine study of US clinics found stem cell treatments cost between about $1,200 and $28,000, averaging roughly $5,100. Spinal and multi-level procedures are likely to sit toward the higher end, and insurance usually does not pay. Compare prices by country in our cost guide and location guides, and check insurance coverage.

Risks and side effects

  • Disc infection (discitis), a serious but rare complication of any injection into a disc
  • Increased back pain after the procedure
  • Nerve injury or spinal fluid leak from needle placement
  • Possible acceleration of disc damage from needle puncture
  • Unregulated products that may be contaminated or contain few living cells

Recruiting clinical trials for back pain

On 4 October 2026, ClinicalTrials.gov listed 2 recruiting or soon-to-recruit stem cell studies for this condition, 2 with sites in the United States. Here are up to eight, latest-stage first:

See all matching studies on ClinicalTrials.gov. A listing is not an endorsement: registration does not mean a study is safe or effective, and genuine trials rarely charge you for the experimental treatment. Ask the study team about costs, placebo groups and follow-up before you enrol. See our clinical trials hub.

Questions to ask before you agree to treatment

  1. Which disc is thought to cause my pain, and how was that confirmed?
  2. Will the injection go into the disc itself, and what imaging guidance will be used?
  3. What sterile precautions are taken to prevent disc infection?
  4. Is this a registered clinical trial, and what happens if my pain gets worse?

Red flags to watch for

  • Claims that injections will ‘regrow’ or ‘rehydrate’ your discs or cure sciatica
  • Treating several spine levels at once without clear evidence that those discs cause your pain
  • Injections into the disc done without X-ray or other imaging guidance

Stem cell therapy for back pain: FAQs

Does stem cell therapy work for back pain?

For pain from one or two worn discs, trials show a small average improvement compared with sham treatment, but experts say it may not be meaningful for many people. Evidence for general back pain, bulging discs or sciatica is very limited. It remains experimental.

How much does stem cell therapy for back pain cost?

US clinic prices vary widely; a 2021 study found stem cell treatments cost from about $1,200 to $28,000, averaging around $5,100. Insurance usually does not cover it.

Is stem cell therapy FDA approved for degenerative disc disease?

No. The FDA has not approved any stem cell treatment for back pain or disc disease, and it warns patients about clinics offering unapproved products.

Can stem cells repair a bulging or herniated disc?

There is no good human evidence that stem cell injections shrink a bulging disc or repair a herniation. Most trials studied disc degeneration, not herniation, and measured pain rather than disc repair.

Is stem cell therapy for back pain covered by insurance?

Generally not. US insurers regard stem cell injections for the spine as investigational, so patients usually pay themselves unless they join a funded clinical trial.
